By signing this document, I acknowledge that I meet the following requirements to be tattooed:

• I am at least 18 years old.

• I am not pregnant and I am not breastfeeding.

• I have not had a bone marrow transplant within the last 12 months.

• I have never received a donor organ.

• I have not had surgery within the last 30 days.

• I understand that tattoo ink can, in some people, cause allergy-like reactions, which may range from skin irritation to, in the worst case, requiring the tattoo to be surgically removed.

• I understand that all tattoos involve certain risks, such as infection, scarring, and touch-ups.

• I understand that if my tattoo contains lettering, typography, dates, numbers, etc., it is MY responsibility to ensure that everything is 100% correct before I get tattooed. The tattoo artist and Tattoo Studio 96 are exempt from liability.

• I have informed the tattoo artist of any conditions/illnesses that may affect the outcome of the tattoo or the act of being tattooed itself, including, for example, epilepsy, bleeding disorders, psoriasis, hives, low blood sugar, diabetes, etc.

• I understand that a tattoo requires special care, etc. I promise to follow the tattoo artist’s instructions in full.

• I am not intoxicated or under the influence of drugs of any kind.

• I have not previously had allergy-like reactions to tattoo ink. If I have, I have informed the tattoo artist.

I agree to all of the above points, and by my signature I confirm that I have read and fully understood this document.
